RN Hospice Coordinator

As an RN Hospice Coordinator at Cleveland Clinic, you will play a crucial role in ensuring patients transition smoothly from hospital to home or facility care. This position is ideal for compassionate nurses who are dedicated to providing high-quality hospice care.

• Act as a member of the interdisciplinary team for service coordination.
• Process inpatient referrals into hospice and coordinate discharge planning.
• Actively educate potential customers about hospice care benefits.
• Ensure patients and families understand the hospice benefit.
• Process inquiries for all new referrals (internal and external).
• Ensure seamless transitions by ordering medical supplies and DME.
• Perform assessments of potential patients alongside the physician.
• Plan delivery of patient care in collaboration with other disciplines.
• Assess patient safety risks, abuse, and neglect.
• Document changes in patient condition and effectiveness of care.

Education:

• Graduate from an accredited school of Professional Nursing
• Bachelor’s of Science in Nursing (BSN) preferred

Experience:

• One year of current clinic experience as an RN
• One year of experience in an acute med/surg or critical care setting

Licenses &

Certifications:

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through AHA or American Red Cross
