Registered Nurse; RN - Home Health Admission - per visit

Overview

BAYADA Home Health Care is seeking a Registered Nurse (RN) for a Home Health Admission role in Independence, Ohio. This full‑time, weekend admission position serves adult and geriatric clients on a per‑visit basis throughout the Northeast Ohio area. Schedule options include Thursday‑Sunday or Friday‑Monday. Home‑health experience is required.

• Follow a designated care plan in accordance with the patient's needs.
• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to Clinical or Client Services Manager.
• Execute and properly document doctors' orders.
• Perform client assessments as necessary.
• Case‑manage and coordinate care.
• Accurately document observations, interventions, and evaluations using a state‑of‑the‑art tablet.

• A current license as a Registered Nurse in OH.
• Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home‑care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
