RN Hospice Coordinator - South Region Hospitals

About This Role

As an RN Hospice Coordinator at Cleveland Clinic, you will support patients transitioning from hospital to home or a facility, ensuring a smooth adjustment and continuity of care. This role is ideal for compassionate caregivers who thrive in a collaborative environment.

• Pay: $30.41 - $46.38/hour
• Location: Independence, OH
• Facility: Cleveland Clinic
• Schedule: Full-time, Day shift

• Act as a member of the interdisciplinary team for coordination of services.
• Process inpatient referrals into hospice and coordinate discharge planning.
• Educate potential customers about the benefits of hospice care.
• Ensure patients and families understand the hospice benefit.
• Process inquiries for all new referrals.
• Ensure seamless transition from hospital to community by ordering medical supplies.
• Perform assessments of potential patients alongside the physician.
• Plan delivery of patient care in collaboration with other disciplines.
• Assess patient for safety risks, abuse, and neglect.
• Document changes in patient condition and effectiveness of care.

• Graduate from an accredited school of Professional Nursing
• Bachelor’s of Science in Nursing (BSN) preferred

• One year of current clinic experience as an RN
• One year of experience in an acute med/surg or critical care setting

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through AHA or American Red Cross
