** Position Overview*
* We are seeking an experienced
** RN Case Manager
** to provide compassionate, patient-centered hospice care throughout the Seven Hills, Ohio area. The RN Case Manager will oversee an assigned hospice caseload, conduct comprehensive patient assessments, develop and manage individualized plans of care, and collaborate with patients, families, physicians, and interdisciplinary team members.
This is a field-based position requiring regular travel between patients' homes during the workday.
** Hospice RN case management experience and Homecare Homebase experience are required.*
* ** Key Responsibilities*
* + Manage an assigned caseload of hospice patients and provide skilled nursing care in the home setting.
+ Conduct comprehensive initial, routine, and ongoing patient assessments.
+ Monitor changes in patient condition and determine appropriate clinical interventions.
+ Develop, implement, and update individualized hospice plans of care.
+ Provide symptom management and end-of-life nursing care.
+ Administer medications and treatments and educate patients and caregivers on medication and symptom management.
+ Provide wound care and other skilled nursing interventions as needed.
+ Educate patients and families regarding disease progression, comfort measures, medications, and hospice services.
+ Coordinate care with physicians, hospice aides, social workers, chaplains, and other interdisciplinary team members.
+ Communicate significant changes in patient condition to appropriate providers and team members.
+ Complete accurate and timely clinical documentation using
** Homecare Homebase (HCHB)** .
+ Participate in interdisciplinary group (IDG) meetings and ongoing care planning.
+ Coordinate medications, medical equipment, supplies, and other services required in the home.
+ Travel between multiple patient homes throughout the workday.
+ Maintain compliance with hospice regulations, organizational policies, and professional nursing standards.
** Required Qualifications*
* + Active
** Registered Nurse (RN) license** .
+ Current
** CPR certification** .
+
** Hospice RN Case Management experience - required.*
* +
** Homecare Homebase (HCHB) experience - required.*
* + Strong clinical assessment, documentation, care coordination, and communication skills.
+ Ability to work independently and effectively manage multiple patient needs.
+ Valid driver's license and reliable transportation.
** Ideal Candidate*
* The ideal candidate is a compassionate, organized RN with strong hospice experience who is comfortable working independently in patients' homes. Excellent clinical judgment, communication, documentation, and time-management skills are essential.

** Pay Details:** $46.00 to $47.36 per hour
