Job Purpose

The Nurse‑Family Partnership (NFP) Home Visitor provides professional nursing services through home visits promoting healthy pregnancies, positive parenting, and child well‑being. This position maintains the highest standards in clinical nursing practice by adhering to the NFP model and to all policies, procedures, and standards of both the NFP and Healthier Moms and Babies.

The Nurse Home Visitor fosters long‑term success for families by delivering education, guidance, and support that strengthen family outcomes and align with HMB’s mission to improve community health.

Nurse‑Family Partnership, a maternal and early childhood home visiting program, fosters long‑term success for moms, their babies, and society. Nurse‑Family Partnership's maternal health program introduces vulnerable parents to caring maternal and child health nurses, helping them achieve healthy pregnancies, become knowledgeable and responsible parents, and give their babies the best possible start in life. Duties and Responsibilities Client Services and Case Management
  • Develop therapeutic relationships and use reflection and motivational interviewing techniques with clients and families in a home visiting environment, modeling a client‑centered, strengths‑based, and solution‑focused approach.
  • Maintain a caseload of no more than 25‑30 clients.
  • Meet with clients and their families, in their home, in accordance with the NFP model and guidelines.
  • Assess physical, emotional, social, and environmental needs of clients and their families as they relate to the NFP domains.
  • Assist clients and their families in establishing goals and outcomes.
  • Provide education, support and referral resources to assist clients and their families in attaining their targeted goals.
  • Coordinate access to community resources.
  • Consult and collaborate with other professionals involved in providing services to women and families.
  • Formulate nursing diagnoses based on nursing assessment and client goals.
  • Evaluate client progress toward stated goals and NFP outcomes.
  • Plan home visits in accordance with client goals and NFP outcomes.
Supervision, Reflection, and Collaboration
  • Meet with the NFP Nurse Supervisor weekly for clinical supervision.
  • Schedule joint home visits with the NFP Nurse Supervisor every four months.
  • Attend and participate in weekly case conferences or staff meetings.
Data Entry, Evaluation, and Quality Improvement
  • Ensure timely and accurate data input in the NFP Management Information System (MIS).
  • Participate in review and analysis of the MIS reports to identify achievements and areas for improvement.
  • Participate in quality improvement initiatives.
  • Maintain confidentiality of all client and organizational information.
  • Document appropriately. Ensure timely and accurate data input and utilization of the web‑based reports and other reports available through NFP.
Organizational Alignment and Representation
  • Follow and support all policies, procedures, guidelines, and standards of NFP and HMB.
  • Participate in community coalitions or meetings to support NFP and HMB initiatives.
  • Promote public awareness of NFP and HMB as requested.
  • Develop and maintain professional community relationships to support client referrals and partnerships.
  • Understand, support, and represent the NFP and HMB mission, vision, and model positively in actions and words to both internal and external customers and colleagues.
Teamwork and Communication
  • Maintain a positive and collaborative work environment and experience for co‑workers and clients.
