- Must reside in Evansville, IN or surrounding area
- Prior experience in healthcare and/or case management
- Bachelor's degree in Social Work, Psychology, Counseling, Gerontology, Nursing, or Health & Human Services with at least two years of relevant experience
- Intermediate to advanced computer skills, including proficiency in Microsoft Word, Excel, and Outlook
- Exceptional communication and interpersonal skills to build rapport with members and stakeholders
- Bilingual (English/Spanish or English/Burmese)
- Experience working with the geriatric population
- Familiarity with Medicare and Medicaid systems
- Knowledge of community health and social service agencies
- Experience in health promotion and wellness coaching
