Overview
A Field Nurse is a registered nurse (RN) who provides healthcare services to patients in non-traditional settings, such as in their homes, schools, or other community locations. The RN will assess, plan, implement, and evaluate individualized care plans in collaboration with individuals, families, and other members of the healthcare team.
Reports to: Deputy Chief of Quality
Responsibilities- Provide in-home skilled nursing assessment care to individuals in accordance with the physician’s plan of care and agency policies.
- Perform comprehensive assessments, including physical, emotional, and social factors.
- Develop, implement, and evaluate patient care plans and update as necessary based on individual’s condition.
- Administer medications and treatments as prescribed and monitor for adverse reactions.
- Educate individuals and families on processes, self-care techniques, and prevention strategies.
- Coordinate care with interdisciplinary team members including physicians, therapists, DSPs, and case managers.
- Document all individual’s care activities accurately and timely in the electronic medical record (EMR).
- Monitor individual’s progress and report changes in condition to the physician and care team.
- Ensure compliance with all local, state, and federal regulations and agency policies.
- Participate in on-call rotation as assigned.
- Current and valid Registered Nurse (RN) license in the state of Indiana.
- Graduate of an accredited nursing program.
- Minimum of one (1) year of nursing experience; home health experience preferred.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Proficient in electronic charting and documentation.
- Excellent written and verbal skills
- Strong clinical and assessment skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage time effectively.
- Compassionate and patient-centered approach to care.
- Knowledge of home health regulations (Medicare/Medicaid, OASIS documentation preferred).
- Frequent travel to patient homes in assigned territory.
- May be exposed to varying home environments and patient conditions.
- Flexible hours based on patient needs and agency scheduling.
While performing the duties of this job, the employee is regularly exposed to (i) individuals residences in which the noise levels are usually moderate and (ii) outside weather conditions.
