Mobile Psychiatric Medication Licensed Practical Nurse

Location:

Community Guidance Center – Indiana

Travel Required.

• Observe and record consumers' behavioral and physical signs and symptoms through regular in‑home assessments.
  • Assist in maintaining adherence to both physical health and psychiatric medications.
  • Formulate Medication Management plans with the participation of the consumer; signed by the consumer and the Community Guidance Center staff.
  • Document in the consumer's electronic health record.
• Increase each consumer's knowledge of their medication's benefits and side effects through a program of ongoing in‑home instruction.
• Assist each consumer in creating an environment that supports medication adherence by enlisting natural supports and increasing motivational and organizational skills.
  • Create lists of medications and allergies to be kept in the consumer's wallet or purse.
  • Suggest ways to coordinate lab work and implement a system for discarding old prescriptions to enhance safety.
• Enhance each consumer's ability to navigate systems of care affecting medication adherence, such as Primary Care Physicians (PCP), labs, specialists, and pharmacies.
• Adhere to seven recognized Sanctuary commitments, which include social responsibility, social learning, non‑violence, open communication, democracy, emotional intelligence, growth, and change.

• Licensed Practical Nurse.
• Act 33/34, FBI clearances and Act 31 certificate.
• One year of clinical experience (preferred).
• At least 2 years of professional experience working in the mental health field (preferred).
• Valid Pennsylvania driver’s license.
• Acceptable Motor Report Vehicle Report.
