Registered Nurse Case Manager – Hospice

Hours: 40 hours per week, 8‑hour shift

• Deliver compassionate, patient‑centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Perform comprehensive patient assessments, symptom and pain management, and individualized end‑of‑life care planning.
• Educate and support patients and families through the end‑of‑life journey, ensuring they feel informed, prepared, and deeply cared for.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently while collaborating with the team and never alone.
• Travel to patient homes within the assigned territory.

• Active Registered Nurse (RN) license in the state of practice.
• Minimum two years of RN experience with clinical expertise to deliver quality patient care; hospice experience preferred.
• Current driver’s license and ability to spend approximately 20% of the day driving to and from patient locations.
