QUALIFICATIONS Education
  • Graduate of an approved school of nursing
Experience/Skills
  • Two years’ experience in physician’s office or related experience (preferred but not required).
  • Experience in Urology preferred but not required
  • Background in medical-surgical nursing and immunizations preferred
  • Knowledge of clinical function
  • Ability to work in conjunction with other clinic personnel
  • Knowledge of community health resources for alternate health services
  • Knowledge of community health nursing principles and practices
  • Ability to communicate effectively both verbally and in written reports
Required Licenses/Certifications
  • LPN or RN Licensure in the State of Indiana
  • CPR

ROUTINE RESPONSIBILITIES Essential Functions-Phone Nurse
  • Conducting patient assessments over the phone by asking detailed questions about symptoms & medical history.
  • Utilizing critical thinking skills to determine the appropriate level of care needed.
  • Strong clinical knowledge and understanding of common medical conditions.
  • Provide medical advice based on the assessment.
  • Directing patients to the right healthcare provider or service.
  • Maintaining accurate documentation of each call.
  • Demonstrate excellent communication skills and empathy towards patients.
  • Updates the patient’s medical and social history, as appropriate.
  • Maintains current knowledge of medications.
  • Accurately documenting all patient interactions including symptoms, advice given, and follow up plans in the electronic medical record.
  • Schedules patients for further testing as directed by the Provider.
  • Secures appointment date and time from specialty referral as directed by the Provider.
  • Sends in prescription refills as directed by the Practitioner.
  • Performs other duties as assigned.
  • Professional demeanor and ability to handle stressful situations calmly.
Essential Functions-Clinic Float
  • Escorts patients to exam rooms and assesses patients per clinic protocols.
  • Obtains and thoroughly documents vital signs, chief complaint.
  • Updates medication lists, and verifies allergies.
  • Updates the patient’s medical and social history, as appropriate.
  • Prepares patient for examination and assists as necessary.
  • Ensure that ordered tests results are in the patient record and available for Practitioner review at time of.
  • Assures adequate patient records are maintained and transferred as requested.
  • Obtains consent for procedures as directed.
  • Collects lab specimens and prepares specimens for transport according to policy.
  • Assists provider in procedures including but not limited to dressing changes, pelvic exams, laceration repairs, injections, and aspirations.
  • Utilizes appropriate aseptic technique.
  • Cleans instruments and prepares them for sterilization according to policy.
  • Maintains current knowledge of medications and their correct administration based on age of the patient and current clinical condition.
  • Follows the five rights of medication administration to reduce the potential for error.
  • Documents in the patient record all medication administered and tests performed in a timely and accurate manner and charges appropriately.
  • Formulates a teaching plan based on identified learning needs and involves patient and family in education.
  • Knowledgeable in growth and development principles and provides age-appropriate care.
  • Schedules patient for further testing as directed by the Practitioner.
  • Keeps track of follow-up labs, wellness checks for future patient reminders.
  • Maintains knowledge of rules and regulations governing the clinic.
  • Sends in prescription refills as directed by the Practitioner.
  • Responds to emergency situations appropriately and immediately.
  • Call 911 as necessary.
  • Performs other duties as assigned.
  • Works in all clinics assigned and needs arise.
