Responsibilities
- Identify patient and family care needs and provide nursing and supportive care for terminally ill patients and their families.
- Maintain the nursing process and initiate and sustain interventions to achieve the highest level of care in collaboration with the primary physician.
- Participate as a member of the interdisciplinary team to develop and implement individualized plans of care and modify care plans based on patient or family response.
- Provide patient and family education to achieve care skills, promote independence, and facilitate full living until death.
- Interpret and teach infection control protocols to patients and families.
- Document care activities accurately and timely in the electronic medical record and on required forms.
- Participate in the Quality Improvement Program to maintain or improve patient care quality.
- Provide direction and supervision to Patient Care Partners, Unit Receptionists, and volunteers to maintain a supportive environment.
- Accurately complete daily acuity assessments for assigned patients.
- Attend to patients and families at the time of death.
- Education:
ADN and/or BSN from an accredited institution. - Experience:
Two years of nursing experience, or an equivalent combination of education and experience. - Skills:
Knowledge of universal precautions for infection control; safe self‑protection practices; strong time‑management; excellent interpersonal and communication abilities; capacity to be patient‑ and family‑focused; competence in using or learning an electronic medical record (EMR). - Licensures/
Certifications:
Current MD registered nursing license.
Job entails frequent standing, sitting, stooping, bending, walking, and lifting. Ability to stand and walk up to ninety percent of work time and lift and reposition patients up to fifty percent of work time. Requires concentration and the ability to perform under stressful situations.
Working ConditionsNormal patient‑care environment with potential exposure to noise, dust, temperature variations, and communicable diseases related to patient‑care activities.
Pay Range$32.18 – $49.46. Final salary offer will be based on qualifications, education, experience, and alignment with organizational needs.
