Description Job Summary

The Certified Nurse Assistant provides basic care under the supervision of the nurse in charge in accordance with our established nursing care procedures.

• Proactively deliver outstanding customer service and care to residents and their families so they feel like they are at home and they are happy.
• Deliver organizational, ethical, mission based and regulatory standards of care and superior personal performance maximizing positive clinical outcomes.
• Assist other members of the team to help them deliver the same standards and superior personal and team performance. Observe and share information with other team members. Approach the job with flexibility and adaptability.
• Meet standards of conduct, appearance, demeanor, continuous learning and personal organization representing the highest level of professionalism.

• Take vital signs (temperature, blood pressure, pulse and respiratory rate).
• Provide direct resident care such as assisting with bathing, eating, dressing, toileting, walking residents, and taking care of incontinent needs.
• Turn and reposition residents who cannot change their own position, to prevent breakdown of their skin.
• Change bed linens.
• Record amount of oral intake and measure urinary output.
• Collect specimens for tests.
• Supply and empty bed pans.
• Interact with residents and family.
• Transport residents and equipment as needed.
• Keep a record of care given.
• Monitor residents and report any variances to normal to the nurse for further assessment.
• Follow infectious disease precautions to prevent the spread of organisms.
• Must assist with the evacuation of residents.

• Participates in departmental, all-staff and on-the-job in-service programs, as scheduled.
• Prepares rooms for resident upon admission; follow admission guidelines.
• Reports all complaints and grievances made by the resident to the Charge Nurse.
• Perform testing and special treatments as instructed.
• Informs Health Care Nurse of supply needs.
• Performs other duties as assigned.

• Follows established safety procedures and precautions while performing tasks and during emergencies.
• Reports all incidents/accidents, unsafe or hazardous conditions, and defective equipment to the Health Care Nurse immediately.
• Assures that work areas are clean and equipment properly stored before leaving on breaks or at end of the workday.
• Assures that Infection Control and Universal Precaution practices are followed while performing duties. Uses appropriate clothing/devices when performing tasks involving the handling of infectious waste and/or blood/body fluids.
• Maintains confidentiality of resident information and honors resident personal and property rights.
• Handles all interactions with residents and staff in a respectful and cordial manner.
• Complies with all Hubbard Hill Estates work rules, policies and procedures.
• Disposes of refuse in accordance with Hubbard Hill sanitation procedures.
• Demonstrates dependability, punctuality, and professional appearance.

Must be certified by Indiana as a nurse assistant. Must be able to read, speak and understand English and follow oral and written instructions.

• Must have sufficient strength to turn, lift, and assist patients with movement and be able to lift, push, pull and move 50 pounds.
• Works throughout the nursing services area.
• Must be able to walk and move about throughout the workday.
• Must be able to see and hear, with or without the use of prosthetics, to adequately meet the requirements of this position.
• Must be available to be on-call and to work beyond normal working hours and on weekends or holidays when necessary.
• Must be able to work with ill, disabled, elderly, emotionally upset or hostile people.
• May encounter frequent interruptions and exposure to infectious waste.
