Licensed Practical Nurse - Rehab Care Center

Job Summary

Under the direction and supervision of an RN/GN, the LPN/GPN is expected to utilize his/her knowledge of practical nursing, age‑specific needs, and the care/treatment of the rehab patients. The LPN/GPN is to be an active member of the interdisciplinary treatment team and work collaboratively in the development, implementation and revision of each patient’s interdisciplinary treatment plan. The LPN/GPN is to observe and evaluate each patient, document his/her observations and evaluations, and provide the RN with relevant clinical information.

• Minimum
  
  Education Required:
  
  Graduate of an accredited LPN program.
• Minimum Experience
  
  Required:
  
  Prefer 1 year of medical‑surgical experience.
• Licensure/Certification
  
  Required:
  
  Current license as a Licensed Practical Nurse in the Commonwealth of Pennsylvania; BLS certification required (or obtain within one month of hire).
• Skills Required:
  
  Commitment to service excellence and performance improvement; nursing knowledge of medical, surgical, and behavioral health nursing; ability to provide direct patient care, observe and document outcomes, and provide patient and family education; excellent writing, computer, and interpersonal skills; ability to work collaboratively as a member of the interdisciplinary team.
