Virtual Clinician Evaluations Nurse Practitioner or Physician Assistant - Indiana

Overview

Signify Health, part of CVS Health®, is hiring full‑time board‑certified nurse practitioners or physician assistants to conduct Video Visits for Medicare Advantage, Medicaid, and select commercial plan members. The role involves delivering a 30‑minute telehealth visit that includes medication and medical history review, education, and answering member questions.

• Conduct online “Video Visits” with health plan members in a private, appropriately equipped space.
• Attend a minimum of 40 hours per week, ensuring 320+ capacity units per month.
• Maintain a visit completion rate of at least 220+ visits per month.
• Adhere to training, affiliation, and compliance requirements for Video Visit delivery.
• Participate in regular manager, team, and clinical meetings.

• Board‑certified nurse practitioner or physician assistant with state licenses in all coverage areas.
• FNP or AGNP certification preferred for NPs.
• Experience with Signify Health In‑Home Health Evaluations, Video Visits, or similar telehealth formats.
• Ability to obtain and maintain multi‑state licenses as required.
• Proficiency with information management and technology systems.

• Credentialed for all health plan clients and lines of business in primary coverage state(s).
• Meet minimum broadband connectivity, video and audio capability, quiet setting, and privacy standards.
• Maintain operational, clinical, and member‑experience key performance indicators.
• Qualified applicants with arrest or conviction records will be considered for employment in accordance with all applicable laws.

• Exceptional verbal, written, and electronic communication skills.
• Ability to work without direct supervision and practice autonomously.
• Proficiency in using a computer screen, iPad, and other electronic devices.

Clinicians deliver Video Visits only, requiring a quiet, neutral background and strict adherence to CVS Health privacy guidelines during remote interactions.
