In-Home Nurse Practitioner or Physician Assistant Per Diem - Jasper

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. This role involves engaging with people in their own homes, conducting medication and medical history reviews, physical evaluations, and ordering diagnostic tests when indicated by the health plan.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad and an intuitive clinical workflow for each evaluation. Prescription of medication, lab test orders, and alterations to current treatment regimens are not required.

• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry, DEE, bone density screening, etc.

The primary workplace is the private residence of health plan members. You must be able to safely perform duties in environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Part time

$37.66 – $81.11 per hour. This pay range represents the base hourly rate for all positions in the job grade within which this position falls. The actual base salary will depend on experience, education, geography, and other factors.
