  Primary Care practice
• Employed position within a nonprofit health system
• Board Certified or Board Eligible in Advanced Practice Provider
• Eligible for state medical licensure and DEA registration
• Access to 3 dedicated exam rooms per provider and dual Sunrise and Athena EMR systems
• Drive clinical excellence by delivering comprehensive, mission-centered primary care to families within the local community

• Critical care hospital environment supporting comprehensive outpatient services
• Access to specialty assets including diagnostic testing, imaging, rehabilitation, cardiology, and dermatology
• Integrated regional referral network within the Ascension St. Vincent Medical Group

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Nurse Practitioner specializing in Prescriptive Authority credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date required.
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

• Master's degree of Nursing required.
