Community Wellness Navigator Program-Intake Coordinator

Job Description

The intake and outreach coordinator leads intake and outreach services for all new walk-in clients and most returning clients to LICI. The coordinator plans and implements intake-related activities and systems, supervise staff and volunteers, ensures goals are met, and satisfies compliance and reporting responsibilities for the assigned actions. The coordinator will work directly with clients in a fast-paced environment while maintaining professional standards and best practices with timely enrollment, referrals to appropriate services, and client communication.

The coordinator also conducts outreach to eligible populations and communities.

The coordinator also conducts outreach to eligible populations and communities.

Intake coordination:

• Oversee and conduct eligibility screenings for walk-in clients and anyone seeking services.
• Maintain up-to-date knowledge and eligibility criteria on all programs offered by LICI, including but not limited to Franklin County funded programs, federally funded programs such as Temporary Assistance to Needy Families (TANF), assistance programs for victims of human trafficking, financial capability services and immigration legal services. Refer eligible clients to appropriate LICI programs using the identified program process.
• Maintain up-to-date knowledge about community resources and refer ineligible clients to community resources such as local Departments of Social Services, shelters, food banks, crisis centers, and others as applicable.
• Develop and maintain effective data tracking systems for intake services, and produce high quality program reports as needed

• Manage a team of staff, volunteers, and interns supporting intake activities in accordance with LICI’s People Manager Standards.
• Comply with agency policies, protocols, and case management best practices.
• Represent LICI in meetings with community partners to provide accurate information about LICI services.
• Conduct outreach to potential clients and community partners about specific LICI programs or services, in collaboration with program managers and senior leadership team.
• Support the delivery of client orientations.
• Other duties as assigned.

• High school Diploma or higher or comparable certification or at least two years in the community
• Ability to interact with community members through written or verbally
• Strong time management and ability to work independently
• Demonstrate ability to be utilize technology
• Strong organizational skills
• Excellent written and verbal communications skills
• Ability to facilitate group meetings
• Available to make home visits and conduct community events
