Accountable for delivery of work product and general collaboration with change and project teams, workstream leads, business owners, and impacted groups during organizational transformation initiatives. Implements and monitors defined change plans through transfer of ownership to business owners. Helps equip stakeholders both internal and external to the project team with the knowledge, skills, resources, and support needed to transition through change while fostering adoption, minimizing disruption, and supporting sustained organizational success.

Implements assigned components of change plans, timelines, and deliverables. Conducts routine change assessments. Tracks progress against assigned milestones and supports reporting on change readiness, adoption, risks, and stakeholder feedback. Supports development of stakeholder engagement plans, adoption plans, resistance prevention and management plans, reinforcement plans, and transition-of-ownership activities. Applies change and project management methodologies such as Prosci, ADKAR, PMP, and Critical Path practices.

Helps monitor change impacts and adoption by gathering feedback, identifying trends, and documenting barriers. Uses change software, project management tools, and data analysis tools to support progress tracking and outcome measurement. Helps stakeholders understand the change, expected impacts, available resources, and adoption expectations. Identifies routine areas of resistance and works with project teams to address challenges. Supports development and implementation of mitigation actions for adoption risks or unexpected setbacks.

Engages with cross-functional teams to gather insight and feedback. Supports alignment of change strategies with project goals and organizational needs. Partners with training, communication, operational, and consulting partners to support a cohesive approach to change. Supports development and delivery of change-related training programs. Assists in preparing stakeholders with knowledge and skills needed to adapt effectively. Maintains and refines tools, templates, process documentation, and support materials.

Provides limited guidance to less experienced Practitioners or project support roles as needed.

• Bachelor's degree in organizational development, Business, Psychology, Healthcare Administration, or a related field is required.
• Requires 3-5 years of relevant experience.
• Certification in Change Methodology is preferred, such as Prosci or APMG certification.
• Certification in Project Management is preferred, such as PMP certification.
• Experience supporting projects, process improvement, communications, training, operations, healthcare, or organizational change initiatives is preferred.
• Foundational exposure to change, project management, stakeholder engagement, or training support is preferred.
• Familiarity with change, project management, data analysis, and virtual collaboration tools is preferred.
