Primary Care Physician (PCP) Responsibilities
- Accountability for outcomes:
The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. - Coaching for health:
The PCP acts as a health coach, helping patients set short- and long-term health goals, partnering with them and following up to improve health outcomes. - Simplifying for action:
The PCP simplifies and prioritizes care to make behavior change more actionable and leads care teams toward performance goals.
- The PCP independently provides care for older adult patients with acute and chronic illnesses, takes full accountability for patient care and outcomes, and seeks specialist consultation when needed while remaining actively responsible.
- The PCP engages with hospitalists and care teams for hospitalized patients and coordinates transitions of care, documenting all details.
- The PCP leads a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office-visiting patients, and collaborates with transitional care teams for homebound or facility-based patients.
- The PCP has an active management role in the center, covers for other providers as needed, and participates in recruiting patients and additional providers.
- Performs other duties as assigned by management.
- Availability:
Accessible to patients through walk-in visits and timely response to phone calls and messages. - Service Orientation:
Provides care that would be desired for a family member or oneself. - Evidence-Based Medicine:
Stays updated on evidence-based medicine, lifestyle and nutrition interventions for improved outcomes. - Physician Leadership:
Continuously develops leadership skills for patients, team, center, and company. - Quality:
Works with care teams to achieve high-quality outcomes and quality measures. - Influence:
Influences patients, teams, and selves compassionately to achieve best outcomes. - Self-Care:
Prioritizes personal well-being to better care for patients. - Proficiency:
Skilled in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and related software. - Travel:
Willing to travel locally, regionally, and nationally for training and recruiting. - Language:
Fluency in English (verbal and written); other language fluency may be required per center. - Independent Judgment:
Uses independent judgment in all aspects of care and administration.
$221,141 - $315,915. Salary depends on experience, education, location, and other factors. Bonus or commission may also be available.
