Job Summary

The caregiver is responsible for providing ADL care and engaging activities for participants while in their home. The caregiver will follow a plan of care for each participant to ensure the optimal level of independence and well-being. The caregiver is responsible for appropriate documentation to include ADL care, housekeeping, errands, engagement of activities and other items provided in the participant’s electronic health record.

Essential

Functions
  • Attend to the daily needs of clients on an individual basis
  • Understands and adheres to established policies and procedures
  • Performs personal care, bathing, and hands on care as assigned
  • Complete housekeeping tasks to include, but not limited to: sweeping, mopping, cleaning bathrooms, vacuuming, errands
  • Provide opportunities for leisure activities and foster independence in the choice and participation of those activities
  • Completes appropriate visit records by the end of each shift
  • Adapt and modify activities to better serve each client
  • Ensure participant safety in the home
  • Provide ongoing support, guidance, role modeling and supervision to clients
  • Provide a supportive environment that includes opportunities for individuals to determine, to the fullest extent possible, personal preferences for daily schedules and routines
  • Maintain knowledge of ongoing issues and developments and inform supervisor of issues and concerns
  • Communicate in a professional manner
  • Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk
  • Remain alert at all times throughout scheduled work shift
  • Participate in core training and maintain certification of required trainings
  • Provide crisis intervention and access emergency services as needed
  • Attends and actively participates in staff meetings
  • Ensure clients we support are treated with dignity and respect in accordance with Independent Adult Day Centers Standard of Conduct Policy
  • Assist the Director of Home Care where needed
  • Other job-related duties as assigned
Qualifications
  • Direct care experience working with geriatric and/or adults diagnosed with Intellectual Disabilities preferred but not required
  • A valid driver's license in state of residence preferred
  • Qualifying criminal background and driving record
  • Ability to pass a drug screen
  • Ability to provide or obtain a 2 step PPD
  • Must be able to lift 50 or more pounds
  • Must be able to demonstrate competence in training areas including CPR, First Aid, non-violent physical interventions, behavior management, ADL care and management, and driving; training provided as needed
  • Ability to calmly and repeatedly model appropriate behavior in difficult situations
  • Ability to react to situations, such as rescue, and prevent an individual from harming themselves

The employee must possess the ability to process vague, abstract verbal and written instructions; work under stress, interruptions, and tight deadlines; problem solve, answer questions and evaluate results of performance; and visualize and assess abstract ideas.
