Human Resources Specialist at Poplar Care Strategies

We are seeking an Activities Assistant/Patient Transport Driver to join our team at Poplar Care Strategies.

The ideal candidate will have a clean driving record, and experience in long term care. The primary responsibilities of this role include transporting patients to and from medical appointments, assisting patients in and out of vehicles, and ensuring their safety and comfort during transportation. Additionally, the Patient Transport Driver will be responsible for scheduling appointments and coordinating transportation logistics.

• CNA license preferred but not required
• Experience in long term care preferred
• Valid driver's license
• Must be at least 25 years old

Job Responsibilities:

• Assisting residents in and out of transportation vehicle.
• Scheduling and attending appointments for residents.
• Establishing and maintaining communication with residents, family members, doctors' offices, and coworkers to improve quality of service.
• Transporting residents out of facility to designated areas in a punctual and timely manner.
