Location: Lagrange
Center Well Home Health is seeking a Home Health Physical Therapist to assess and treat patients in their homes, planning and administering prescribed physical therapy to restore function and independence. You will document progress and guide a patient-centered rehabilitation plan, reporting to the Clinical Manager and care team.
You will evaluate strengths, develop rehab programs, supervise PTAs and HHAs, document clearly, and communicate patient progress to family and physicians.
