Full-Time Physical Therapist

Heartland Home Health is looking for a Full‑Time Physical Therapist to join our team in Ft. At. This is a full‑time position with a schedule of Monday through Friday, 8:00 am‑5:00 pm, and a sign‑on bonus of $5,000.

• Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
• Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
• Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
• Document all visits, assessments, and progress notes accurately and in a timely manner.
• Support patients in maximizing their independence and improving quality of life—wherever they call home.

• Master’s degree in Physical Therapy from an accredited program.
• Current PT license in the state of employment (or eligibility to obtain).
• Valid driver’s license, auto insurance, and reliable transportation.
• 1+ year of experience in a hospital, home health, or outpatient rehab setting.
• Experience in in‑home therapy delivery.
• Experience in geriatric, orthopedic, or neurologic rehab.
• Experience in post‑acute care or chronic condition management.
• Experience working as part of a mobile or interdisciplinary care team.
