Heartland Home Health is looking for a Full‑Time Physical Therapist to join our team in Ft. Wayne, IN. We provide skilled rehabilitation services that empower patients to recover, restore function, and live safely at home.

Schedule

Monday–Friday, 8 am–5 pm. Salary, full benefits, 401(k) match, Sign‑On Bonus – $5,000.

What You’ll Do
  • Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
  • Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
  • Adjust treatment plans and techniques as needed to achieve optimal outcomes.
  • Collaborate with the interdisciplinary home‑health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
  • Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
  • Document all visits, assessments, and progress notes accurately and in a timely manner.
  • Support patients in maximizing their independence and improving quality of life—wherever they call home.
About You
  • Master’s degree in Physical Therapy from an accredited program.
  • Current PT license in the state of employment (or eligibility to obtain).
  • CPR certification required.
  • Strong communication, assessment, and problem‑solving skills.
  • Ability to work independently in a variety of home environments.
  • Valid driver’s license, auto insurance, and reliable transportation.
Preferred Experience (Not Required)
  • 1+ year of experience in a hospital, home health, or outpatient rehab setting.
  • In‑home therapy delivery.
  • Geriatric, orthopedic, or neurologic rehab.
  • Post‑acute care or chronic condition management.
  • Working as part of a mobile or interdisciplinary care team.
