Elara Caring is seeking an Occupational Therapist-PRN to join our home-based care team in Indiana. You will help patients regain independence through compassionate therapy delivered in the home setting, with autonomy and support from a dedicated team.
You will conduct initial evaluations, create Plans of Care, ensure OASIS compliance, and educate patients and families on home programs. Travel up to 50% within the service area (Valparaiso, Merrillville, Portage, Wheatfield).
