Hospice Registered Nurse

Hospice Social Worker - Gentiva Join Gentiva, a leading home-based care provider, as a Hospice Social Worker supporting patients and families in their homes. In this role, you will complete psychosocial assessments, develop individualized care plans, and provide counseling, crisis intervention, and grief support at end of life. You will coordinate community resources, educate families on hospice services, and collaborate closely with an interdisciplinary team to promote dignity, comfort, and patient-centered goals.

Responsibilities

• Conduct comprehensive psychosocial assessments for hospice patients in their homes
• Develop and implement individualized care plans addressing emotional, social, and resource needs
• Provide counseling, crisis intervention, and grief support to patients and families
• Educate families on hospice philosophy, goals of care, and community resources
• Collaborate with nurses, physicians, chaplains, and other team members in IDT meetings
• Advocate for patient dignity, autonomy, and culturally sensitive care
• Coordinate community services, financial and legal resource referrals as needed
• Maintain accurate, timely EMR documentation in compliance with hospice regulations
• Participate in quality improvement and case review activities
• Support bereavement programs and follow-up services for families
