Summary

Provide person-centered support to older adults and individuals with disabilities in program, community, and transportation settings. This role assists persons served with daily living skills, individualized goals, socialization, community involvement, health and safety needs, and personal independence.

The Direct Support Professional – Habilitation works as part of a team to implement Person-Centered Individualized Support Plans (PCISP) while maintaining a safe, respectful, and engaging environment.

Key Responsibilities
  • Deliver direct support and instruction based on each person’s PCISP, goals, needs, preferences, and abilities
  • Create meaningful, age-appropriate activities that encourage participation, socialization, independence, and community inclusion
  • Assist persons served with personal care needs, including toileting, dressing, eating, hygiene, and mobility support as needed
  • Support skill development in communication, behavior, self-care, leisure, social interaction, and community integration
  • Promote personal autonomy, self-direction, individual rights, dignity, and choice-making for each person served
  • Assist persons served with participation in community activities, recreational opportunities, restaurants, appointments, and other community resources
  • Complete daily documentation of progress toward goals, services provided, accidents, incidents, behavior concerns, and other required reporting
  • Monitor and report changes in behavior, health, safety, or unusual situations to supervisory staff promptly
  • Administer or dispense medications when trained, approved, and authorized to do so
  • Assist with assessments, case conferences, staff meetings, program planning, and quality‑assurance activities
  • Assist with the care and maintenance of agency equipment and report repair needs as appropriate
  • Maintain billing documentation, daily billing forms, Medicaid waiver tracking forms, production records, and other required agency records
  • Handle approved purchases, petty cash, and persons served financial records responsibly and according to agency procedures
Transportation Responsibilities

When assigned transportation duties, the Direct Support Professional – Habilitation will:

  • Safely transport persons served to and from activities, appointments, community events, and assigned pick‑up/drop‑off locations
  • Complete required transportation logs, trip records, daily vehicle inspections, and vehicle maintenance documentation
  • Ensure all persons served are accounted for before departure and follow scheduled pick‑up and drop‑off times
  • Ensure seat belts, car seats, and other required safety devices are available and used correctly
  • Safely load and unload persons served, including non‑ambulatory individuals when applicable
  • Report vehicle damage, accidents, injuries, maintenance concerns, and safety issues immediately to the appropriate supervisor
  • Follow all agency transportation policies, traffic laws, DOT requirements, and safety procedures
  • Maintain a current first‑aid kit and transportation manual in the assigned vehicle
Qualifications & Skills
  • Ability to develop and maintain professional relationships with persons served, families, co‑workers, and community partners
  • Ability to work independently while also collaborating effectively as a member of a team
  • Ability to complete accurate documentation, reports, billing forms, and service records
  • Ability to use word‑processing, database, and spreadsheet software
  • Commitment to person‑centered services, confidentiality, safety, and individual rights
Physical & Work Requirements
  • Ability to lift and carry up to 50 pounds for a distance of 15–20 feet
  • Ability to bend, stoop, and assist persons served with mobility, transfers, and daily living needs
  • Ability to safely perform physical assistance tasks as required by each person’s needs
