Organization OverviewMartin Center, Inc. (d.b.a. Martin Center Sickle Cell Initiative) is a nonprofit human services organization dedicated to aiding and enhancing the lives of individuals and families affected by Sickle Cell Disease (SCD) and associated disorders throughout Central and Southern Indiana.
Our programs focus on education, advocacy, outreach, care coordination, and support services that improve health outcomes and reduce barriers to care. Through partnerships with healthcare providers, schools, community organizations, and public health agencies, we strive to provide comprehensive, culturally responsive services to those impacted by Sickle Cell Disease.

Pay Rate: $22.00/Hour
Benefits:
This position is not eligible for employee benefits or paid holidays.
Work Schedule:
Approximately 20–25 hours per week, with occasional evening and Saturday hours as needed to support community outreach, health fairs, educational programs, support groups, and agency-sponsored events.
Location:
Indianapolis, Indiana, with travel throughout Central and Southern Indiana as needed for client outreach, meetings, health fairs, and community activities.

The Part-Time Community Health Worker/Client Follow-up Specialist plays a vital role in supporting the Martin Center’s mission by conducting newborn screening follow-up, client outreach, education, care coordination, and community engagement.
Working closely with the President & CEO and other team members, this position helps identify, educate, and connect individuals and families affected by Sickle Cell Disease and other hemoglobinopathies with appropriate healthcare and community resources.
The successful candidate will work with confidential health information and must maintain strict compliance with HIPAA (Health Insurance Portability and Accountability Act) regulations.
Essential Duties and Responsibilities- Conduct outreach to families of newborns identified through newborn screening with Sickle Cell Trait, Sickle Cell Disease, or other hemoglobinopathies.
- Provide culturally appropriate education regarding Sickle Cell Disease, Sickle Cell Trait, available community resources, and the importance of ongoing medical care.
- Conduct follow‑up activities to ensure families understand the diagnosis and are connected with appropriate healthcare providers.
- Locate and engage individuals living with Sickle Cell Disease who may benefit from Martin Center programs and services.
- Coordinate referrals and assist clients in accessing healthcare, transportation, financial assistance, social services, and other community resources.
- Maintain accurate and timely client records in the agency's client management system while ensuring confidentiality and HIPAA compliance.
- Collaborate with hospitals, healthcare providers, the Indiana Newborn Screening Program, public health agencies, schools, and community organizations to improve access to care and services.
- Participate in meetings related to newborn screening, Sickle Cell Disease initiatives, and other program activities.
- Represent Martin Center at health fairs, educational presentations, community outreach events, support groups, and awareness campaigns.
