Role Description

The Clinical Pharmacist is a full-time, on-site role based in Indianapolis, IN, responsible for providing safe, effective, and evidence-based pharmaceutical care to patients across inpatient and outpatient settings. Day-to-day responsibilities include reviewing and verifying medication orders, performing clinical assessments, monitoring therapy for efficacy and safety, and collaborating with physicians, nurses, and allied health professionals on individualized treatment plans. The role involves preparing and overseeing compounding of medications when needed, counseling patients and caregivers on appropriate medication use, and supporting transitions of care to minimize medication-related problems.

The Clinical Pharmacist also contributes to quality improvement initiatives, participates in interdisciplinary rounds, assists with formulary management, and may provide education to learners and staff in alignment with Eskenazi Health's teaching mission.

Qualifications
  • Demonstrated competency in Pharmacy Practice and Pharmacy operations, including dispensing, verification, and adherence to regulatory and institutional policies.
  • Strong foundation in Pharmaceutical Care and Medicine, with the ability to assess medication therapy, interpret clinical data, and optimize treatment outcomes.
  • Experience with Compounding, including sterile and non-sterile preparations, following USP and institutional standards.
  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy; completion of a PGY1 pharmacy residency preferred.
  • Current, unrestricted pharmacist license (or eligibility for licensure) in the state of Indiana.
  • Proficiency in clinical decision-making, medication safety principles, and use of electronic health records and pharmacy information systems.
  • Effective communication and collaboration skills, with the ability to work in interdisciplinary teams and provide clear patient and caregiver education.
  • Commitment to serving diverse and vulnerable populations and contributing to teaching and quality improvement activities.
