Professional Case Management is seeking a compassionate Caregiver to provide in-home support to clients with complex medical needs. In this Personal Care & Service role, you will assist with daily living activities, personal hygiene, mobility, meal preparation, light housekeeping, and companionship while following individualized care plans. You'll work closely with a clinical team, observe and report changes in client condition, and help maintain a safe, dignified home environment.

Responsibilities
- Provide in-home personal care assistance including bathing, grooming, dressing, toileting, and hygiene support
- Assist clients with safe mobility, transfers, and positioning, using proper body mechanics and equipment
- Observe and report changes in clients' physical or emotional condition to supervising clinicians or case managers
- Follow individualized care plans and instructions from nurses and other healthcare professionals
- Support basic health-related tasks as permitted, such as assisting with prescribed exercises and reminders for medications
- Prepare simple, nutritious meals and assist with feeding as needed while following dietary guidelines
- Perform light housekeeping tasks such as laundry, bed making, and maintaining a clean and safe home environment
- Assist with errands and appointments, including occasional transportation support, as appropriate
- Provide companionship, emotional support, and social engagement to promote client well-being and independence
- Accurately document care provided, client status, and any incidents in accordance with company and regulatory standards
- Follow infection control, safety, and confidentiality policies at all times
- Collaborate with the care team and participate in ongoing training to maintain and improve caregiving skills
Required Skills
- Personal care and activities of daily living (ADL) assistance
- Safe patient handling, transfers, and mobility support
- Basic knowledge of vital signs and monitoring changes in condition
- Infection control and basic hygiene practices
- Meal preparation and nutrition awareness
- Home safety and fall-prevention practices
- Clear verbal and written communication
- Time management and reliability
- Documentation and basic record-keeping
- Use of durable medical equipment (e.g., walkers, wheelchairs, oxygen equipment)
- Professionalism and confidentiality (HIPAA awareness)
- Empathy, patience, and conflict de-escalation
