Role Description

The Hospice Medical Social Worker MSW is a full-time, on-site role based in Lakewood, NJ. This role provides psychosocial assessments and ongoing emotional, social, and resource-based support to hospice patients and their families. Daily responsibilities include developing and implementing individualized care plans, coordinating community resources, assisting with discharge planning and transitions of care, and providing counseling related to grief, coping, and end-of-life decisions.

The Medical Social Worker collaborates closely with the interdisciplinary team, including physicians, nurses, chaplains, and aides, to ensure comprehensive, patient-centered hospice care. The role also involves documentation of services, participation in care conferences, and advocacy for patients’ rights and preferences.

Qualifications
  • Strong foundation in Medical Social Work and hospice Social Work, with the ability to address complex psychosocial needs in end-of-life care.
  • Skills in Case Management and Discharge Planning to support smooth care transitions and coordination of services.
  • Experience in Social Services, including connecting patients and families with community resources, benefits, and support programs.
  • Master’s degree in Social Work (MSW) from an accredited program; current social work license in the state of New Jersey or eligibility for licensure.
  • Prior hospice, palliative care, or medical social work experience strongly preferred.
  • Ability to work collaboratively within an interdisciplinary team and communicate effectively with patients, families, and colleagues.
  • Demonstrated empathy, cultural sensitivity, and commitment to providing non-discriminatory, person-centered care.
  • Strong organizational and documentation skills, with proficiency in electronic health records and basic computer applications.
