Colonial Nursing and Rehab is seeking a compassionate and dedicated Occupational Therapist (OT) to join our skilled nursing and rehabilitation facility in Crown Point, IN. In this role, you will deliver high-quality therapy services designed to improve residents' independence, function, and overall quality of life.

• Part-Time

• Implement individualized treatment plans developed in collaboration with the rehabilitation team
• Conduct thorough evaluations and establish measurable short- and long-term therapy goals
• Educate residents, families, and caregivers on techniques that support ongoing progress
• Work closely with interdisciplinary team members to coordinate comprehensive care
• Monitor resident progress and adjust treatment plans as appropriate
• Maintain accurate and timely clinical documentation

• Active Occupational Therapist license (OTR/L) in Indiana or associate’s degree from an accredited OT program
• Commitment to patient-centered care
• Strong communication and interpersonal skills
• Ability to work effectively as part of an interdisciplinary team
• Previous skilled nursing or rehabilitation experience preferred
