Direct Support Professional – In-Home Services
Provide individualized, in-home and community-based support to individuals with disabilities. This role focuses on promoting independence, developing life skills, and supporting individuals in achieving personal goals while remaining in their home environment.
· Assess individual needs and assist in developing and implementing individualized service plans
· Provide training in daily living skills, including personal care, communication, and social development
· Support individuals in accessing community resources and participating in community activities
· Assist with household tasks, organization, and maintaining a safe home environment
· Escort individuals to medical appointments and community activities as needed
· Support individuals with correspondence, budgeting, and bill paying when necessary
· Maintain accurate documentation of services, progress, and outcomes in compliance with regulations
· Develop materials and maintain supplies to support individual goals
· Act as a resource to families, caregivers, and other team members
· Promote independence, self-advocacy, and personal growth
· Maintain positive relationships with individuals, families, and community partners
· Remain flexible to meet the needs of individuals served
· Strong interpersonal and communication skills
· Ability to work independently in home and community settings
· Empathy and ability to support individuals with diverse needs
· Strong organizational and documentation skills
· Ability to collaborate effectively with team members and families
· Basic computer skills (word processing, spreadsheets, and data entry)
· Ability to lift and carry up to 50 pounds
· Frequent bending, stooping, and assisting individuals with mobility
· Ability to travel between client homes and community locations
· Valid driver's license, reliable transportation, and proof of insurance required
· CPR and First Aid certification
· Preventing Disease Transmission (PDT)
· Certified Restraint Training (CRT)
· Medication Administration (Core A &
B)
· Completion of required trainings
High School Diploma or equivalent preferred.
Experience working with individuals with disabilities is required.

$350 Sign-on Bonus eligible position. Must be scheduled for a minimum of 20 hours/week to be eligible for the sign-on bonus.
