Cardiology Clinical Pharmacy Specialist – Ambulatory

This position is located at our South Hospital. As part of a very progressive ambulatory care pharmacy team at Community Health Network, you will participate in team‑based care under collaborative drug therapy agreements, making medication changes as appropriate. You will manage hypertension, dyslipidemia, cardiovascular risk reduction, anti‑arrhythmic monitoring, tobacco cessation, DOACs and VKA anticoagulation services, and provide transitional care management for cardiology discharge patients both in‑person and virtually.

Other responsibilities include staff education, guideline and policy development, participation in performance improvement initiatives, and practice advancement. You will have opportunities to mentor and teach pharmacy students and residents and serve as a clinical resource for the healthcare team. Your expertise will advance patient care outcomes in the General Cardiology Clinic, Home Blood Pressure Monitoring Program, Electrophysiology and Heart Failure Clinics, and DOAC/Anticoagulation Clinics.

The role supports expansion of cardiology pharmacist services to the Indianapolis region.

• Strong communication skills and a collaborative approach to patient care.
• Practice at the top of your license utilizing collaborative drug therapy management agreements.
• Provide individual patient education (in‑person and remotely) focusing on high‑risk patients (heart failure, coronary artery disease, hypertension, dyslipidemia, arrhythmias, venous thromboembolism, LVAD, smoking cessation).
• Work closely with physicians and health‑care teams to provide direct patient care and overcome medication adherence issues.
• Assist patients in obtaining cost‑effective medications through medication assistance coordinators.
• Serve as a drug information resource for providers and clinic staff.
• Monitor drug‑therapy‑related adverse events and order appropriate clinical or laboratory tests.
• Engage in population health projects to impact patient lives.
• Understand quality improvement principles.
• Preceptor of pharmacy students and residents.
• Graduate of an accredited college of pharmacy (Bachelor or PharmD);
  Doctorate degree.
• Indiana Pharmacist license.
• Successful completion of a PGY1 Pharmacy Residency and a PGY2 residency with cardiology or ambulatory care focus (strongly encouraged) or at least three years working under a drug therapy agreement in a clinical setting.
• Board certification as a Board Certified Pharmacotherapy Specialist, Board Certified Ambulatory Care Pharmacist, Board Certified Cardiology Pharmacist, or Certified Anticoagulation Care Provider (required within two years of accepting the position).
