As a Care Coordinator with Adult & Child Health / Brightli , you will play a key role in supporting individuals receiving behavioral health and substance use treatment. You’ll work closely with clients, families, clinicians, and community partners to coordinate services, remove barriers to care, and help clients achieve meaningful progress in their recovery and overall wellbeing.

• Review initial assessments and identify client needs related to healthcare, housing, employment, education, social support, and legal systems

• Coordinate outpatient behavioral health and substance use services

• Assist clients and families in accessing community resources and supportive services

• Advocate for clients and help remove barriers to care

• Participate in treatment team meetings and clinical staffings

• Coordinate referrals to outside providers when additional services are needed

• Represent the organization professionally within the community

• Support the development and ongoing updates of individualized treatment plans

• Ensure accurate and timely documentation of all services provided

• Address and correct any chart documentation deficiencies

• Assist clients and families with transition and discharge planning

• Provide crisis intervention and support when needed

• Help coordinate transportation and scheduling for treatment services

• Ensure clients receive appropriate referrals for medical or additional support services

Successful candidates will demonstrate:

• Knowledge of case management and rehabilitation practices in behavioral health or substance use treatment

• Understanding of community resources and support systems

• Ability to collaborate with multidisciplinary teams

• Strong communication, organization, and documentation skills

• Ability to advocate effectively for client needs

Preferred qualifications include:

• Bachelor’s degree in a human services field , such as:

Experience working with behavioral health, substance use treatment, or community-based services is strongly preferred.

• Successful completion of background screening including criminal, driving, abuse/neglect, and fingerprint checks
