Peabody Retirement Community is seeking a compassionate, organized, and resident-focused Social Services Coordinator to join our Skilled Nursing team.
This position works closely with residents, families, nursing, and the interdisciplinary care team to support residents’ psychosocial needs and overall well-being. Responsibilities include assisting with care planning, resident and family meetings, MDS documentation, discharge and relocation planning, behavioral health needs, community resources, and helping residents and families successfully navigate the long-term care experience.
We’d love to hear from candidates with experience in any of the following areas:- Social Services Designee (SSD) certification or training
- Social services or resident services in long-term care
- Skilled nursing, assisted living, or senior care
- Social work, human services, psychology, sociology, gerontology, nursing, or a related field
Previous long-term care experience is strongly preferred. Strong communication, organization, documentation, and interpersonal skills are essential.
