This individual supports the intention to improve the quality of life for older adults through developing and maintaining relationships, activities and assistance within the community. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Demonstrate strong communication skills using ICARE and the Awareness Wheel when needed;
Engage with senior participants and providers in a way which shows sincere interest and dedication to your role;
Assess eligibility for referred persons and complete Pet Connect enrollment as appropriate;
Enter client information and other data for Pet Connect program participants;
Coordinate with community partners for the Pet Connect program as needed to maintain relationships and meet participants needs;
Deliver pet food and supplies to eligible participants with the intention of creating an exceptional experience;
Monitors supply for Pet Connect program and is fiscally responsible. Orders, tracks and completes purchase requests for all supply orders;
Understands and meets requirements of grants and participates in applying for new grant opportunities for Pet Connect as needed;
Assist Meal Site representatives and other department staff with enhancing activities provided at congregate meal sites;
Assists with planning activities and events provided to participants in any Meals & More programs;
Encourage participants to take part in meaningful activities by role modeling participation;
Serve as a CICOA ambassador and maintain professionalism in the community. Supervisory Responsibility No supervisor responsibility.

Education and/or Experience A high school diploma is required. Experience in developing and maintaining effective relationships with seniors, and an interest in empowering older adults to engage in meaningful activities and programs that add value, foster independence, and enhance their quality of life.
