Shady Nook Nursing and Rehab is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team in Lawrenceburg, IN. This flexible PRN opportunity is ideal for a skilled clinician who enjoys helping residents improve strength, mobility, balance, and independence while providing high-quality, patient-centered care in a supportive environment.

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide evidence-based, patient-centered therapy interventions to improve mobility, strength, balance, and functional independence
• Monitor resident progress, update goals, and modify treatment plans as appropriate
• Collaborate with the interdisciplinary care team to support coordinated and effective rehabilitation outcomes
• Maintain accurate documentation and ensure compliance with clinical and regulatory standards
• Educate residents, families, and caregivers on exercises and strategies that support continued progress and independence

• Licensed Physical Therapist (PT) in the state of Indiana
• Current BLS certification
• Strong clinical judgment and communication skills
• Compassionate, patient-focused approach to care
• Ability to work independently while collaborating effectively with the rehabilitation and interdisciplinary teams
