Job Title: Community Engagement & Participant Support Coordinator (Part-Time)

Reports To: Director of Operations or Program Manager

Position Summary

The Centers of Wellness for Urban Women (CWUW) is seeking a Part‑Time Program Coordinator to support Sisters Together:
Move More, Eat Better, a community‑based wellness program focused on chronic disease prevention, nutrition, physical activity, diabetes prevention, blood pressure management, and whole health for women.

This position is ideal for someone with experience in public health, community engagement, nonprofit programs, health education, social services, or community health work. The Program Coordinator will play an important role in helping women improve their health and well‑being through education, support, community connections, and access to resources.

The successful candidate will be responsible for coordinating program activities, recruiting and engaging participants, facilitating workshops and wellness activities, supporting community outreach efforts, maintaining program records, collecting outcome data, and assisting with grant reporting and evaluation activities.

Key Responsibilities Program Coordination & Facilitation – 35%
  • Coordinate and support implementation of Sisters Together program activities.
  • Facilitate health education workshops, wellness activities, support groups, and community‑based programs.
  • Lead or support movement activities such as walking groups, stretching sessions, and wellness challenges.
  • Assist participants in setting health goals and developing action plans to improve their overall well‑being.
  • Support participant recruitment, enrollment, retention, and follow‑up activities.
Community Engagement & Outreach – 30%
  • Represent CWUW at community events, health fairs, partner meetings, and outreach activities.
  • Conduct outreach to increase awareness of program services and recruit participants.
  • Build and maintain positive relationships with community organizations, healthcare providers, and neighborhood partners.
  • Identify opportunities to expand program visibility and community impact.
Data Collection, Evaluation & Grant Support – 20%
  • Maintain accurate participant records, attendance logs, and program documentation.
  • Collect participant surveys, assessments, and outcome data.
  • Track program performance measures and support grant compliance activities.
  • Assist with monthly, quarterly, and annual grant reports.
  • Document participant success stories and program outcomes.
  • Support continuous quality improvement efforts through data collection and evaluation.
Participant Support – 15%
  • Connect participants to healthcare, wellness, and community resources.
  • Provide follow‑up support to encourage engagement and goal achievement.
  • Assist women in navigating barriers that impact their health and well‑being.
Qualifications Required
  • Minimum of two (2) years of experience in public health, health education, community outreach, community health work, social services, nonprofit programming, or a related field.
  • Experience facilitating workshops, trainings, support groups, or community‑based programs.
  • Experience recruiting, engaging, and supporting program participants.
  • Strong organizational, communication, and relationship‑building skills.
  • Experience collecting data, maintaining records, and documenting program outcomes.
  • Proficiency with Google Workspace (Docs, Sheets, Forms, Drive).
  • Ability to work independently while managing multiple priorities and deadlines.
Preferred
  • Community Health Worker (CHW) certification or willingness to obtain certification.
  • Associate's or Bachelor's degree in Public Health, Health Education, Social Work, Human Services, Nonprofit Management, or a related field.
  • Experience supporting grant‑funded programs and grant reporting.
  • Knowledge of diabetes prevention, hypertension management, nutrition, physical activity, and health equity.
  • Familiarity with Indianapolis communities and community resources.
  • Experience with Canva, Zoom, and social media platforms.
