About the roleSeeking highly skilled, compassionate, and professional Private Duty Nurses (LPN or RN) to join our home health agency in a full‑time, weekday capacity to provide care and support for clients in the west/southwest metro area of Indianapolis, IN.
Details- Immediate PDN care is needed for an adult client in west/southwest Indianapolis (Zip Code: 46241, near Lynhurst, Maywood, Decatur, Bridgeport).
- Full‑time, weekdays (40 hours/week):
Monday – Friday 8 AM – 4 PM. - Requires experience with G‑tube care.
- Mileage reimbursement between multiple clients is provided.

- RN Hourly: $37 – $40
- LPN Hourly: $28 – $30
* Starting wage rates are based on experience, credentials, positive employment history & references, and overall fit.
Performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of the professional organization and agency. All functions are performed in accordance with established policies, practices, and the state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.
Reports to:
RN Case Manager;
Clinical Administrator/Director of Nursing.
- Graduate of an accredited nursing school.
- Current license to practice as a Licensed Practical Nurse or Registered Nurse in Indiana.
- Minimum of one (1) year of experience in a home health or acute care setting, or equivalent; 1+ years of in‑home care preferred.
- Paid experience with G‑tube care and toddlers required.
- Active CPR and First Aid certifications required.
- Effective written and verbal communication skills.
- Good interpersonal communication skills.
- Current driver’s license or state , and reliable transportation.
- Desirable character traits: reliable, honest, professional, compassionate, detail‑oriented (in care & documentation), accountable, friendly, respectful, positive.
- Provides professional nursing care under the direction of a Registered Nurse and communicates client status changes to the RN per agency guidelines.
- Performs skilled nursing visits for clients as directed by the RN Case Manager and follows the established care plan.
- Recognizes and reports changes in client condition to the Case Manager and physician as directed.
- Communicates information in a timely manner, consistent with agency policy and urgency of the situation; includes verbal updates and accurate documentation.
- Scope of practice includes personal care, assessment, medication monitoring & administration, client teaching, emergency intervention, and nursing services within professional scope and as delegated by the Case Manager.
- Licensed LPN M‑IV may perform venipuncture for lab blood samples.
- Promotes personal safety and a safe environment for clients and coworkers.
- Demonstrates knowledge of safety/infection control practices and compliance with policies.
- Recognizes and responds to potentially unsafe situations.
- Demonstrates safe use of equipment and makes recommendations to improve safety.
- Performs work in compliance with agency policies and professional/community standards.
- Accepts responsibility for the professional nursing role.
- Follows applicable state and federal laws and agency standards.
- Attends required meetings.
- Maintains confidentiality and refrains from discussing client or staff identities or conditions publicly or with unqualified persons.
- Participates in development, implementation, and evaluation of the Agency Quality Improvement Program and related activities.
- Performs other related duties as necessary.
Frequent standing, walking/bending/stooping; occasional sitting. Able to push/pull 10‑20 lbs occasionally, 20‑30 lbs infrequently; lift 5‑10 lbs occasionally, 10‑30 lbs infrequently.
