Field-Based | Full-Time | $2,500 Sign-On Bonus

What You'll Do

As a Home Health LPN, you'll provide skilled, compassionate care while helping patients remain safe and independent at home:

* Deliver nursing care based on the Plan of Care under RN and physician direction

* Provide patient-centered education and support for self-care

* Monitor, document, and communicate changes in patient condition

* Collaborate closely with RN Case Managers and interdisciplinary teams

* Promote patient safety and help prevent unnecessary hospitalizations or ER visits

* Build trusting relationships with patients, families, and caregivers

* Accurately document visits, medications, and outcomes

* Uphold the highest standards of professionalism and compassion

Job Requirements:

* Graduate of an accredited nursing program (Certificate, Diploma, or Associate)

* Current State LPN/LVN License

* 1 year of clinical experience (home health or hospice preferred)

* Comfort with independent field-based work

* Valid driver's license, reliable transportation, and insurance

* Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs

* Ability to travel (approx. 50%) and meet physical job requirements
