Position Type:
Full-time, recruiting for an Inpatient Palliative Care Advanced Practice Provider (APP). Scope of Care:
Provide holistic care, addressing the physical, functional, psychological, and spiritual needs of individuals with serious illnesses. Patient-Focused Approach:
Utilize a person-family-centered approach to care, focusing on symptom relief and alleviating the stress and burden associated with a disease process. Responsibilities:
Perform thorough history and physical examinations on patients. Timely dictation of appropriate hospital reports. Establish presumptive diagnoses, order necessary laboratory studies, and manage patient problems under physician supervision. Initiate and coordinate laboratory, radiological, and special examinations or tests required for evaluation. Provide counseling and instruction on patient problems, with referrals to other services as needed. Offer follow-up care in accordance with the physician's plan of care.

Coordinate care with discharge planners for transitions to Nursing Homes, etc.

Qualifications:

Experience:

Prior experience with end-of-life and chronic illness is preferred. Demonstrated compassion for patients facing serious illnesses. Strong communication skills to effectively interact with patients and their families. Licensure:
Possess an Indiana Medical License.

Certifications:
