Registered Nurse; RN - Outpatient Mental Health

Overview

Outpatient Mental Health (MH) Registered Nurse (RN) executes position responsibilities to improve patient and organizational outcomes. The psychiatric MH RN provides patient‑centered and recovery‑oriented care, maintains self‑awareness while caring for patients living with MH conditions, and facilitates care coordination.

• Facilitate continuity of care, link with the Inpatient MH Units, participate in interdisciplinary treatment team huddles and meetings, warm hand‑offs, and develop content as well as facilitate nurse‑led psycho educational groups.
• Use therapeutic communication and de‑escalation techniques, practice recovery‑oriented language, and provide Veterans, their families and significant others with education regarding illness, medications, and treatment plan.
• Assist or direct safety concern resolution, collaborate with program leadership to improve patient and program outcomes, and direct team response to psychiatric and medical emergencies.
• Document nursing triage assessments, risk screens, measurement‑based care tools, clinical reminders, and patient plans of care according to facility policy.
• Maintain professional boundaries, provide constructive feedback to peers, support colleagues through knowledge sharing, and foster a safe, supportive environment conducive to professional development.
