Job SummaryAltra Home Health is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team. In this fulfilling role, you will provide individualized, high-quality care to both pediatric and adult patients in their homes. Many of our patients have complex medical needs, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and, at times, require tracheostomy or ventilator support. You will play a key role in promoting safety, comfort, and quality of life, allowing patients to receive the care they need in the place they feel most at ease—home.
Responsibilities- Perform thorough head-to-toe assessments and follow individualized plans of care.
- Provide care that may include G-tube feedings, medication administration, wound care, trach care, and ventilator support.
- Care for patients with professionalism and empathy.
- Educate patients and caregivers on disease management, emergency procedures, and therapeutic techniques.
- Collaborate closely with physicians, therapists, and other team members to optimize care.
- Document all care activities accurately and promptly in compliance with agency and state requirements.
- Maintain rigorous safety, infection control, and privacy standards.
- Shift durations range from 4 to 12 hours
, depending on patient needs and nurse availability. - Flexible scheduling options available for daytime, evening, and weekend shifts.

- Active LPN license in Indiana or an active compact LPN license.
- At least 1–2 years of clinical nursing experience (home health and pediatric experience strongly preferred).
- Experience with G-tube, trach, or vent care preferred (but training will be offered).
- Current CPR certification.
- Strong critical thinking and independent decision-making abilities.
- Excellent interpersonal and communication skills.
- Reliable transportation and a valid driver’s license.
