Wellness Nurse / Healthcare Coordinator (RN or LPN) Overview

We are seeking a compassionate and motivated Wellness Nurse / Healthcare Coordinator (RN or LPN) to join our growing team. In this rewarding, community-based role, you will support individuals with complex medical needs by coordinating care, monitoring health, and promoting overall wellness.

• Perform nursing assessments and develop individualized care and wellness plans
• Provide ongoing health monitoring and chronic disease support
• Coordinate care with physicians, therapists, and interdisciplinary teams
• Oversee and assist with medication management and monitor side effects
• Conduct in-home and community visits (frequency based on client needs)
• Train and support caregivers and Direct Support Professionals (DSPs)
• Maintain accurate documentation, including progress notes and reports
• Assist with health-related aspects of ADLs/IADLs when needed
• Ensure compliance with Indiana BDDS waiver program requirements
• Participate in care reviews, incident reporting, and quality improvement

• Current, active Indiana RN or LPN license (required)
• Valid driver’s license and reliable transportation
• Strong organizational, communication, and problem‑solving skills
• Ability to work independently in community‑based settings
• Experience in case management, home health, or long‑term care preferred
• Experience with developmental disabilities or behavioral health (a plus)
