We are seeking a Licensed Practical Nurse (LPN) to join our team and work closely with our case management staff to support coordinated, patient-centered care.

Key Responsibilities:

• Assist with patient assessments and monitoring
• Collaborate with case managers and care team members
• Perform medication set-ups (med-set ups) and administer medications as ordered
• Document patient status and report changes
• Educate patients and caregivers on care plans and medications

Qualifications:

• Active LPN license in good standing with the Indiana State Board of Nursing
• Compliance with Indiana licensing requirements (IPLA), including renewal and scope of practice
• Strong communication, organization, and teamwork skills
• Experience in home health or case management preferred
