is seeking a Nurse Practitioner or Physician Assistant to join ourFamily Medicine team at our Epler Parke clinic!

• Full-time, Day Shift
  • Offering a 5 or 4 day work week
  • No weekends, holidays, or call, allowing for work/life balance
• Team includes 7physicians and 1 otheradvanced practice provider

Key Responsibilities:

• Assess health status through obtainment of relevant health history and comprehensive physical exam, preventative screening, and identification of medical risk factors.
• Formulate appropriate differential diagnosis and identification of individual, family, and community needs.
• Develops treatment plan to maximize the health potential of the individual, family, and/or community.
• Orders and adjusts medication per collaborative written agreement and prescriptive authority in accordance with the Indiana Nurse Practice Act, or in consultation, and with signature of physician.
• Serves as a clinical resource for the nursing staff, physicians, hospital, and other healthcare providers.
• Serves as an advocate for patients, staff, and management.
• Manage acute and long-term care of the patient.

Qualifications:

• Master's degree required
• FNP or PA certification required
• 2-3 years of outpatient experience in primary care or urgent care highly preferred
