Work Shift: Monday – Friday, 8:00 am to 5:00 pm local time and rotating on-call

Seeking Nurse Practitioners licensed in New York. The role is primarily virtual full‑time except required in‑person training sessions. Candidates must not be in a reasonable commuting distance from the posting location unless accommodation is provided.

• Provides primary and urgent health care via telephone and video modalities to patients receiving home/community‑based services through state Medicaid programs, dual eligible members and other membership as assigned by our MCO partners.
• Develops and implements clinical plans of care for adult patients facing chronic and complex conditions (co‑morbidity, limited resources, chronic illnesses).
• Gathers history, performs physical exam and diagnostics as needed, and develops treatment plans based on patient goals and current conditions.
• Identifies and closes gaps in care.
• Meets patients’ and family’s physical and psychosocial needs with support from the company’s inter‑disciplinary team.
• Educates patients and families about medication usage, side effects, illness progression, diet and nutrition, medication adherence and crisis anticipation and prevention.
• Maintains contact with other clinical team members, patients’ other physicians and providers to coordinate optimal care and resources for the patient and family in a timely, consistent with state regulations and company policies.
• Maintains patient medical records and documentation consistent with state regulations and company standards.
• Participates in continuing education as required by state and certifying body.
• Prescribes medication as permitted by state prescribing authority.

• MS in Nursing.
• Active, national NP certification.
• Valid, current, active and unrestricted Family or Adult Nurse Practitioner license in the state of New York.
• Valid, current, active RN license.
• 2+ years of experience managing complex care cases.
• Experience working with Electronic Medical Records (EMR).

• Possession of DEA registration or eligibility.
• Experience managing complex care cases for developmental disabilities and chronically ill patients.

Salary Range: $121,088 to $198,144 for this position in New York, Illinois (depending on experience, education, location).
