Licensed Practical Nurse; LPN

Signature HealthCARE is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. As an LPN with Signature HealthCARE, you will play a vital role in delivering high-quality, resident‑centered nursing care in a supportive, innovative environment focused on revolutionizing healthcare. Working closely with registered nurses, physicians, therapists, and other members of the interdisciplinary team, you will provide direct hands‑on care to residents and patients, helping them achieve their highest practicable level of health and well‑being.

In this role, you will perform comprehensive and ongoing assessments of residents, accurately obtain and record vital signs, and observe and report changes in condition in a timely and professional manner. You will administer medications and treatments in accordance with physician orders and facility policies, ensure safe and accurate documentation in the electronic medical record, and maintain strict adherence to infection control, safety, and quality standards.

You will provide wound care, catheter care, tracheostomy care, and other nursing procedures within the scope of LPN practice and state regulations, and you will collaborate in the development and implementation of individualized care plans.

You will serve as a key clinical resource and role model for certified nursing assistants and other support staff, offering guidance, direction, and feedback to promote excellent resident care. Educating residents and their families about health conditions, medications, and care routines is an important part of this position, and you will do so with patience, empathy, and respect. You will participate in care conferences, shift reports, and interdisciplinary team meetings, contributing observations and recommendations that support resident goals and the overall plan of care.
