JOB TITLERN Case Manager (PRN / As Needed)
KEY RESPONSIBILITIES- Mix of case management, recertifications, SOC, ROC, other assessments and services, occasional Skilled Nursing visits, new hire orientations, trainings, in-services, supervisory reviews.
- Perform client assessments, direct, plan, initiate appropriate actions in home care situations.
- Manage/supervise a team of RN, LPN, HHA, and Personal Care Aides.
- Document, communicate, coordinate patient status/change.
- Promote personal safety and a safe environment for clients and coworkers.
- Travel surrounding counties to meet service area requirements.
- At least 1 year of Case Management experience and home health agency (or equivalent) experience.
- Experience with OASIS and NON-OASIS assessments.
- 1+ years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.
- Experience and willingness to serve clients of all ages.
- Graduate of an accredited School of Nursing.
- Minimum 1 year as RN, active RN license in Indiana.
- Minimum 1 year of experience in home health or acute care, 1+ years paid home health experience required, 2+ years preferred.
- Minimum 1 year of experience in case management.
- CPR and First Aid certifications required.
- Experience in supervisory role, wound care, lab draws, and OASIS assessments a plus.
- Strong background in nursing, patient care, confidentiality.
- Strong attendance and dependability.
- Desirable traits: reliable, honest, professional, compassionate, detail-oriented, accountable, friendly, respectful, positive.
- Familiarity with HIPAA regulations and medical terminology.
- Valid Driver's License and reliable transportation.
& SCHEDULE
PRN or Part-Time with opportunity for long-term employment. Primarily Weekdays/Daytime (M-F), early evening and rare weekend shifts available. No on-call requirements or overnight shifts. Flexible schedule around personal commitments. No overnight travel.
SERVICE AREA & COMMUTEIndianapolis, IN and surrounding 2-hour radius. Primary service area centered in Indianapolis and Metro, typical travel 10-60 minutes, occasional up to 2 hours required.
COMPENSATION & BENEFITS- Hourly Wage: $35 - $40/hour, paid weekly on Fridays.
